Welcome to Hospice - Gift of Love

Skilled Nursing Services: Our licensed Registered Nurses perform a variety of highly skilled nursing services as directed by the patient’s physician. Our registered nurses are care givers and case managers; they teach and advise the client and family members, coordinate the treatment plan with other team members, and communicate with the client’s physician and family.

Certified Wound Specialist (also known as a WOCN): An RN with a special certification in wound and Ostomy care. Credentialed WOC Nurses have successfully met rigorous criteria and passed a national board examination to be certified in the area(s) of wound, ostomy, and/or continence nursing.

Physical Therapy:: Our licensed physical therapists provide education to, and treatment of, patients experiencing acute onsets or flair-ups of various neurological, bone or muscular disorders. After a complete evaluation, and consultation with the client's physician, our physical therapists develop a personalized and consistent rehabilitation program utilizing a variety of treatment techniques. When appropriate, our physical therapists provide family or caregiver training to reinforce and enhance the benefits of treatment to ensure better, faster recoveries.

Occupational Therapy: Our licensed occupational therapists provide activities and instruction designed to increase safety and independence in daily activities as well as functional use of client’s arms and legs.

Speech Therapy: Our certified speech therapists are an integral part of the rehabilitation team, with expertise in management of communication and swallowing disorders.

Medical Social Work: Social work services are a vital component of our home health care program. Our goal is to improve or maintain the social, emotional, functional, and physical health status of each client while enhancing the coping skills of the family and other caregivers. Social Workers may be able to help in the following areas: advocacy with community agencies, information about, and referral to, programs such as Meals on Wheels or non-skilled Aide services, counseling and crisis intervention, application for financial assistance with long-term care planning, and placement counseling.

Home Health Aide Services: Our aides are trained and certified health care workers who provide assistance to patients with personal care.
